1. What is a Shopify post-purchase survey?

A Shopify post-purchase survey is a short set of questions shown right after checkout, most often on the Thank you page. Most surveys ask 1–3 multiple-choice questions that take seconds to answer.

The answers are zero-party data. Customers give you this information on purpose, so you don’t have to guess it from clicks or cookies.

The survey runs before the order arrives. So it asks about the buying decision, not about the product itself.

2. Why run a post-purchase survey on Shopify

A post-purchase survey shows you 4 things your analytics can’t:

  • The sales your ad dashboard misses. Since April 26, 2021, Apple has required apps on iOS 14.5 and later to ask permission before tracking users, so many sales no longer link back to an ad. A buyer who answers “a friend told me” reveals a word-of-mouth sale that no pixel would record.
  • The friction that almost cost you the sale. Baymard Institute found that 40% of US shoppers who abandoned a cart, excluding those “just browsing,” left because extra costs like shipping, tax, and fees were too high. If your buyers name the same issue, you know where to start improving your Shopify conversion rate.
  • Why people buy. Answers like “a gift” or “I ran out” tell you which message and email flow each buyer needs.
  • What to sell next. “What else do you wish we sold?” gives you product requests from people who already pay you.

3. Where post-purchase surveys can appear

Post-purchase surveys can appear in 5 places: the Thank you page, the Order status page, the post-purchase page, email, and SMS. Each place suits a different kind of question.

Shopify’s documentation confirms that the Thank you page is a one-time page, while the Order status page can be visited many times. Shopify’s checkout apps guide limits the post-purchase page to one app at a time.

Most stores start with the Thank you page, because every buyer sees it. Email and SMS suit questions about the product itself. If you already text customers, your Shopify SMS marketing flow can carry a one-question survey after delivery.


4. Best Shopify post purchase survey apps

Shopify has no native way to run a post-purchase survey, so you need a third-party app. Shopify’s list of Thank you page customization options includes no survey feature. The free Shopify Forms app only builds pop-ups and inline forms on storefront pages.

The plan requirement is low. According to Shopify’s checkout apps guide, apps that add blocks to the Thank you and Order status pages work on the Basic Shopify plan and higher. Apps that change the information, shipping, and payment steps of checkout require Shopify Plus.

Every app below adds its survey as a block in Shopify’s checkout and accounts editor. Shopify asked stores to replace old scripts on these pages with apps, and Shopify’s upgrade notice says August 26, 2026 was the final deadline. Stores that missed it were upgraded automatically, so any guide that tells you to paste survey code into “Additional scripts” is out of date.

Stores with a developer can also build a custom survey with Shopify’s checkout UI extensions.

I checked each app’s Shopify App Store listing on October 2, 2026. Prices change often, so confirm them before you install.

A few details sit behind the table:

  • Fairing includes unlimited questions and responses on every plan, plus NPS surveys.
  • Grapevine carries Shopify’s “Built for Shopify” badge and also runs surveys by email and on Shopify POS.
  • Zigpoll also runs exit-intent, product page, email, and SMS surveys, so you can hear from shoppers who didn’t buy.
  • KNO limits its $19 Starter plan to 1 survey with 3 questions. The $119 Analyst plan raises that to 2 surveys with up to 999 questions each.
  • Okendo sells Surveys as one product inside its platform, next to Reviews, Loyalty, Quizzes, and Referrals. Okendo’s pricing page lists unlimited surveys and responses, plus post-checkout surveys.

Choose your app by these 4 factors:

  • Order volume. Fairing is free under 200 orders a month. Above that, Grapevine’s flat $25 costs less than Fairing’s $49 tier.
  • Number of questions. KNO’s Starter plan caps you at 3 questions. Fairing and Grapevine have no question limits.
  • Who you want to hear from. Pick Zigpoll if you also want answers from visitors who leave without buying.
  • Tools you already use. Okendo users keep survey data in the same customer profile as their reviews. All 5 apps list Shopify Flow and Klaviyo integrations.

5. Post-purchase survey questions to ask

The best post-purchase survey questions each change a decision you will make. If no possible answer would change what you do, cut the question.

Ask in this order: attribution first, motivation second, friction third. The attribution question is the easiest to answer, and it gives most stores the data they need first.

#1 Attribution questions

“How did you hear about us?” is the standard attribution question. Use multiple choice, so answers are easy to count.

Build the answer list with care:

  • List your real channels, such as Instagram, TikTok, Google search, and email.
  • Add channels no pixel can track, like a friend or family member, a podcast, and in-store events.
  • Include an “Other” option with a text field, so new channels can show up.
  • Shuffle the answer order if your app offers it, so the list order doesn’t steer answers.

#2 Motivation questions

Motivation questions tell you why the order happened today. Two work well:

  • “What made you buy today?” Options: a sale, a gift occasion, running out, a recommendation, other.
  • “Who is this order for?” Options: myself, a gift, my family.

#3 Friction questions

A friction question asks what almost stopped the sale. Try “Was anything close to stopping you from ordering today?”

Offer options like shipping cost, delivery time, sizing, payment options, and “nothing.” If buyers mention sizing, a clear size chart on your product pages answers that question before the next shopper has to ask it.

#4 Satisfaction and NPS questions

Satisfaction questions work best by email after delivery. A Thank you page survey runs before the customer has the product.

Net Promoter Score (NPS) is the most common satisfaction question. Bain & Company, which created NPS, asks “How likely are you to recommend us to a friend or colleague?” on a 0–10 scale.

Scores of 9–10 are promoters, 7–8 are passives, and 0–6 are detractors. Your NPS equals the percentage of promoters minus the percentage of detractors.

6. Best practices for more responses and cleaner data

The best post-purchase surveys are short, easy to answer on a phone, and placed where buyers see them first. These 7 practices cover the details.

  • Keep it short. SurveyMonkey studied about 100,000 surveys. Respondents spent about 75 seconds on the first question, including any intro, and about 30 seconds per question from question 3 to 10.
  • Watch the time limit. In the same study, surveys that took longer than 7–8 minutes saw completion drop by 5–20%. People were less patient with customer surveys than with work or school surveys.
  • Open with one multiple-choice question. A single tap is the easiest way to start. Save open-text questions for the end.
  • Place the survey first on the Thank you page. Shopify’s checkout apps guide allows up to 3 app blocks in each area of a page, so order matters. If you also run a post-purchase upsell, drag it below the survey. A store using BOGOS’s Thank You Page Upsell, for example, can move that block under the survey in the same editor.
  • Ask new and returning buyers different questions. New buyers can tell you how they found you. Returning buyers already answered that, so ask them about the product or what to sell next.
  • Design for mobile. Keep answer options short enough to fit on one phone screen, and skip grid-style questions. Preview the survey on your own phone before you publish it.
  • Use rewards with care. A review by Singer and Ye (2013) found that surveys offering incentives get significantly higher response rates than surveys without them. Keep any reward small, and compare answers from rewarded and unrewarded buyers before you trust the data.

Review your questions every quarter. Retire a question once its answers stop changing, and test a new one in its place.


7. What to do with post-purchase survey answers

Survey answers help only when they change a decision. Use them in 4 ways: adjust ad budget, fix friction, target offers, and plan products and emails.

#1 Move ad budget with care

Use attribution answers to check your ad dashboards, not to replace them. Answers are self-reported, so buyers may forget a channel or name the last one they saw.

Compare at least 2 periods before you move money. A single week can mislead if one promotion or one influencer post brought a spike of traffic.

A channel that buyers name often but your dashboard barely credits deserves a closer look. Test a small budget increase there, then check whether total sales follow.

#2 Fix the friction buyers mention

Friction answers give you a fix list. Start with the issue buyers mention most.

Shoppers also need to see the threshold. A cart progress bar shows them how far they are from free shipping as they add items.

#3 Turn answers into customer tags and targeted offers

Customer tags let you act on survey answers automatically. Shopify Flow can add a tag to a customer, and Shopify’s Flow documentation includes a template that tags customers with their “How did you hear about us?” answer from a survey app.

All 5 apps in the table above list Shopify Flow as an integration. That means you can tag customers by any answer, such as “gift” or “repeat buyer.”

8. Mistakes and limits to watch

Post-purchase surveys have 3 built-in limits and 3 common setup mistakes. Know them before you trust the data.

The 3 limits:

  • Only buyers answer. This is called survivorship bias: you hear from people who bought, not from people who left. Pair your survey with an exit-intent popup that asks leaving visitors one question, and with a Shopify heat map that shows where non-buyers stop.
  • Answers depend on memory. Buyers may name the last place they saw you, not the first. Treat attribution answers as a direction, not an exact count.
  • Small samples mislead. A burst of answers after one promotion can skew results. Compare several weeks before you act.

The 3 mistakes:

  • Asking too many questions. Long surveys lose people, as the SurveyMonkey data above shows. Keep the Thank you page survey to 1–3 questions.
  • Keeping an old script-based survey. If your survey lived in “Additional scripts,” check that it still appears after Shopify’s checkout upgrade. Rebuild it as an app block if it doesn’t.
  • Burying the survey. A survey placed below several other blocks is easy to miss. Put it first on the page.
